Understanding Pain – Why Does Chronic Pain Persist?
If you live with chronic pain, you already know that it is more than just a physical injury. Pain is a complex experience shaped by your body’s chemistry, your nervous system, your diet, your sleep, your stress levels, and much more.
When pain first begins — from an injury, arthritis, a nerve problem, or another condition — your body sends pain signals from the site of the problem through your spinal cord and up to your brain. Along the way, your nervous system can either turn the volume of those signals up or turn it down. In chronic pain, the body’s natural “volume control” system often becomes unbalanced, allowing pain signals to be amplified even when the original injury has partially or fully healed.
This means that treating chronic pain effectively requires more than just treating the condition causing the pain. It also means helping your body restore its natural ability to manage pain signals — and that is exactly what the 4-Domain Approach to Pain is designed to do.
What Is the 4-D Chronic Pain Approach?
The 4-D Approach is a conceptually new approach to managing chronic pain that goes beyond conventional assessments and medications. It is built. on the understanding that the severity of the chronic pain experience is not just based on the extent of damaged nerves or injured tissues — it is also driven by how the nervous system processes pain signals originating from painful condition and communicated within the central nervous system and brain.
To provide a comprehensive approach to managing pain therefore requires treating both the pain condition as well as how the pain signals from damaged tissue are processed. The 4-D protocols are not intended to replace conventional management of chronic pain, but to complement it.
Acknowledging that the pain experience can be modified in other ways such as through surgery, behavioral techniques and mindful exercises, the 4-D protocols are based mostly on the use of natural compounds (nutraceuticals) largely derived from foods found in the anti-inflammatory diet. The Four Domains (or the 4 Demons!)
Conditions that influence the severity of pain can be divided into four pathologic biological processes, or domains: Systemic Inflammation, Neuroinflammation, Oxidative Stress and Mitochondrial Dysfunction:
- Systemic Inflammation — Diffuse cellular and organ inflammation that drives pain signals
- Neuroinflammation — Inflammation in the brain and spinal cord that makes nerves overly sensitive
- Oxidative Stress — Cell and nerve damage from an imbalance of harmful “oxidizing” molecules
- Mitochondrial Dysfunction — Reduced cellular energy production that also increases nerve excitability
These 4 Domains (or 4 Demons) are responsible not just for magnifying pain, but they are also major contributors to the risk of developing diabetes, heart disease, stroke and Alzheimer’s. The 4-Domain (4-D) approach addresses these conditions through the use of natural supplements derived from foods and plants that make up the anti-inflammatory diet.
Importantly however, the 4-D approach is not limited to taking supplements, the foundation of the 4-D approach requires lifestyle adjustments, including diet, exercise, sleep, and stress reduction – the most important components of the protocol. Everyone is affected by all four domains but each individual has a unique “demon profile”. Blood tests and questionnaires help identify which domains are most active for an individual, so treatment is tailored to the individual.

The Foundation: Getting Started with Tier 0
Before creating one’s personalized plan, every patient begins with a set of foundational steps called Tier 0. These are universal — they benefit all chronic pain patients regardless of their specific pain condition or domain profile. Think of Tier 0 as building a strong foundation before constructing the rest of the house.
Tier 0, the Foundation, has two main components: lifestyle enhancement and nutraceuticals. It begins with emphasizing the important role of one’s lifestyle and it’s contribution to chronic pain. A healthy lifestyle may be described as being built on four pillars:
- Diet
- Exercise.
- Sleep
- Stress Reduction
Part 1: Lifestyle Enhancement — The Four Pillars
Nutraceuticals alone are not enough. Research consistently shows that lifestyle factors are among the most powerful tools for managing chronic pain. The 4-D Protocol incorporates four lifestyle pillars that are started immediately and reinforced throught the protocols. These for lifestyle pillars impact heavily on all four domains.
Anti-Inflammatory Diet
What one eats directly affects all four of the domains – it feeds the demons or it suppresses them.
An anti-inflammatory diet emphasizes:
- Vegetables, fruits, whole grains, nuts, and seeds
- Fatty fish (salmon, sardines, mackerel) — natural sources of omega-3s
- Olive oil, avocado oil and other healthy fats
- Herbs and spices, especially ginger
An anti-inflammatory diet minimizes:
- Ultra-processed foods (packaged snacks, fast food, sugary drinks)
- Refined sugars and white flour
- Excessive red meat and processed meats
Important caveat: the anti-inflammatory foods do not balance with the pro-inflammatory foods, they are not created equally. What this means is that pro- inflammatory foods weigh more heavily against one – they create more damage than a similar amount of anti-inflammatory foods may correct.
It can be accurately said that the true benefit of sticking to an anti-inflammatory diet is that it avoids the pro-inflammatory foods that are so damaging.
One doesn’t not need to attempt a perfect diet overnight. Small, consistent changes add up over time and can meaningfully reduce your pain. The best and simplest start is to cut back and gradually eliminate sugary drinks, such as sodas, especially those with High-Fructose Corn Syrup (HFCS). HCFS is found almost everywhere – check product labels – and it stands out as one of the most inflammatory and unhealthy foods out there. A devil general in the demon army.
Exercise
Physical activity is one of the most effective treatments for chronic pain — even though it may seem counterintuitive when one is hurting. Exercise helps by:
- Activating the body’s natural pain-relief systems
- Reducing inflammation
- Improving the energy production in your cells
- Boosting mood and reducing stress
Your exercise plan will be tailored to your current abilities. The goal is to start where you are and gradually build up. Even gentle walking, stretching, or water-based exercise counts.
The goal is to work toward 150 minutes per week of moderate activity, but any increase from your current level is beneficial.
Sleep
Poor sleep and chronic pain create a vicious cycle — pain disrupts sleep, and poor sleep makes pain worse. Improving sleep is a high priority in the 4-D Approach. Steps include:
- Establishing a consistent sleep schedule
- Creating a cool, dark, quiet sleep environment
- Limiting screens before bedtime
- Using melatonin (a foundational supplement) to support natural sleep
- Screening for sleep disorders such as sleep apnea if indicated
The goal is 7–9 hours of restorative sleep each night.
Stress Reduction
Chronic stress directly worsens pain by keeping the nervous system in a heightened state of hyperactivity. It triggers hormonal changes that contribute strongly to inflammation and heightens the perception of pain severity. Even brief daily stress-reduction practices can make a measurable difference:
- Deep breathing exercises (5–10 minutes daily)
- Meditation or mindfulness practice
- Gentle yoga or tai chi (Work a physical therapist to help find an approach that fits
- Cognitive behavioral techniques for pain management
- Social connection and enjoyable activities
- Don’t forget Quality Dog Time!
The goal is 10-15 minutes at least per day of stress-reduction activity

Part 2: Foundational Supplements
As part of the foundational assessment, blood levels of key vitamins and minerals are assessed. Research shows that people with chronic pain are more likely to have sub-optimal levels of certain nutrients, and correcting these deficiencies can provide meaningful reduction of pain.
Here is what is included and why:
Many people with chronic pain have low vitamin D levels. Vitamin D helps regulate the immune system and reduce inflammation. Studies show that correcting vitamin D deficiency can significantly reduce pain. Blood level swill be checked, and a dose will be prescribed to bring you into the optimal range (40–60 ng/mL). Vitamin D3 is very affordable and widely available, often covered by insurance.
Omega-3 fatty acids, found naturally in fish and seafood, are among the most well-studied natural anti-inflammatory compounds. They help your body produce special molecules called “resolvins” that actively calm inflammation rather than just blocking it. Multiple large studies have shown that omega-3 supplementation reduces chronic pain, with benefits that increase over time. A blood test will be used to guide dosing. IOmega-3 is often covered by insurance.
PEA is a natural compound that your body already makes in small amounts. It works by calming overactive immune cells in your nervous system — the same cells that contribute to neuroinflammation and amplified pain signals. Studies have shown that PEA reduces pain across many different pain types, including nerve pain, joint pain, and widespread pain conditions like fibromyalgia. It has an excellent safety profile with no significant side effects reported in clinical trials. A special ultra-fine (ultramicronized) form is used for better absorption.
One may know melatonin as a sleep aid, but it also has direct pain-relieving properties. Melatonin works on specific receptors in the brain’s pain control center, helping to restore the body’s natural ability to turn down pain signals. It also protects cells’ energy-producing mitochondria. Studies show that melatonin significantly reduces chronic pain while also improving sleep quality — and poor sleep is one of the most common factors that worsens chronic pain.
Magnesium is a mineral involved in hundreds of processes in the body, including nerve function. It acts as a natural blocker of a receptor in the nervous system (called the NMDA receptor) that, when overactive, amplifies pain signals — a process known as central sensitization. Many chronic pain patients are magnesium-deficient. A specific form called magnesium L-threonate is preferred because it is uniquely able to reach the brain in spinal cord, where it can directly support healthy nerve function.
B-Complex Vitamins (with Methylation Support)
B vitamins are essential for nerve health, brain chemistry, and a process called methylation that affects how genes are expressed. Some people carry a common genetic variation (MTHFR) that makes it harder for their body to use regular folic acid. For these individuals — and for chronic pain patients in general — methylated (pre-activated) forms of B vitamins are recommended. One’s clinician may prescribe a medical food called ENLYTE, which contains high-dose methylated folate and other B vitamins. If cost is a concern, an over-the-counter methylated B-complex is an effective alternative.
Synbiotics (Probiotics + Prebiotics)
Gut health has a direct connection to pain. The bacteria in the digestive system communicate with the brain and immune system through what scientists call the “gut-brain axis.” An unhealthy gut can increase inflammation and make the nervous system more sensitive to pain. Synbiotics — a combination of beneficial bacteria (probiotics) and the fiber that feeds them (prebiotics) — help restore a healthy gut environment and may reduce pain and improve mood and sleep.

What Happens After Tier 0?
Once foundational supplements and lifestyle changes are in engaged (typically over the first 4–8 weeks), and blood test completed, depending on one’s response to the initial interventions, and lifestyle progress, Tier 1, nutraceuticals will be introduced. Based on one’s unique 4-D profile, additional targeted supplements may be added in a stepwise fashion to address the specific biological processes that are contributing most to one’s pain.
The 4-D Protocol is not a quick fix — it is a comprehensive, personalized approach that works with the body’s own systems to reduce pain over time. Many patients notice improvements within the first few weeks, with continued benefits building over months.
